The Bridge: Client Care Center, our case management center, offers year around support through a collaborative process to assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet the client’s health and service needs to transition out of homelessness.Bridge to Home’s goals include identifying a permanent shelter location and expanding services to provide comprehensive care. As one of the fastest-growing communities in the United States, the Santa Clarita Valley will continue to be challenged to fund the social services required by a rapidly developing society. With public funds increasingly scarce for social programs, Bridge to Home has taken a primary role in meeting the needs of the hungry and homeless in this community. Housing or Rent Assistance for Santa Clarita residents - Rental Assistance: One time, move-in, rental assistance; Eviction prevention: Housing retention funds for those who are at risk of losing their housing; Referrals for other housing, such as: Veteran, Shared, Low income, County resources and subsidized programs; Case Management - Client assessment to identify service needs, development of care plan, service referral, monitoring and follow up; Employment assistance: Resume preparation, interview clothing/job supplies. Free bus pass for job search; California I.D. or license reduced fee voucher; Life Skills Services: Money management, social skills, independent living and self-sufficiency; Optical program: Assists with free eye exam, prescription and glasses; Grooming services: Free haircut services and hygiene supplies; Referrals for dental, medical and substance abuse services; Free voucher for clothing, shoes and coats
